Introduction:
Vulval swelling is not an uncommon presentation. There are huge number of causes like Preeclampsia, Allergy, Cellulitis, Vulvovaginitis, Vulval abscess, Bartholin cyst, trauma, Tuberculosis, syphilis, filariasis, lymphogranuloma venerum, tumours etc. diagnosis and treatment based on cause, site, onset, duration, associated symptoms and investigation findings. however syphilitic vulval swelling is a rare case.

Case report
A 45 years menopausal divorcee lady, para 1, from low socioeconomic condition, who worked as commercial sex worker before marriage (20 years back) came with vulval swelling for five years, skin rash with itching and patchy alopecia for same duration.
Examination reveals she is of average built, nutritional status bellow average, mildly anaemic, afebrile, normotensive. Several skin lesions found in different part of body including nasal mucosa which was dry, scally in some area and wet with small pustule in other part. She also had purulent discharge from right ear.
