Abstract
The aim of this study was to compare the effectiveness of carbetocin and oxytocin when these drugs were administered after caesarean section for prevention of post-partum haemorrhage (PPH). This comparative study was conducted on 100 women with singleton pregnancy from April 2014 to September 2014 in the department of Obstetrics and Gynecology, North East Medical College Hospital, Sylhet, Patient with hypertension, preeclampsia, epilepsy, cardiac, renal or liver disease were excluded. Patients were divided into two equal groups as Group-A and Group-B. Patients in Group A received a bolus dose of 100 µg carbetocin IV immediately following delivery of the baby. Patients in Group B received 10 IU oxytocin IM immediately following delivery of the baby followed by 20 IU of oxytocin in 1000 ml of IV infusion (150 ml/hour). Haemodynamic effects of both drugs and the need for additional uterotonic agents were evaluated. In addition, hemoglobin level between two groups of patients was compared. Regarding the haemodynamic aspect, both drugs were found to have hypotensive effect, though blood pressure reduction was found greater in Group-B. Additional uterotonic agents were significantly needed in Group-B patients (24.5% vs. 0%, p<0.01). No significant difference was found in both group of patients regarding blood loss and in haemoglobin level (p>0.05).
Keywords: post-partum haemorrhage, uterotonic drugs, carbetocin, oxytocin.
