Abstract
This was a prospective, interventional and comparative study conducted in the Department of Orthopedics, Sylhet MAG Osmani Medical College Hospital, Sylhet, Bangladesh during the period from July 2008 to June 2010 with a view to find out the difference of the outcome between Locking Plate System (LPS) and Dynamic Compression Plating (DCP) in the management of Humeral Shaft Fracture For this purpose 34 patients with fracture shaft of the humerus were selected according to inclusion and exclusion criteria; and randomly distributed into two groups as LPS group and DCP group, each consisting of 17 patients. Two patients in each group failed to complete 24 week follow up. The mean age ( 38.07 plus/minus 17.17 years in LPS versus 38.6 plus/minus 11.94 years in DCP: p = 0.922 ) and sex [11 (73.3%) male in LPS versus 8 (53.3%) male in DCP (p = 0.45) ) did not differ significantly between two treatment group. Infection [2 (13.3) %versus 3 ( p = 1.0001 latrogenic radial nerve palsy [0 (0.0%) versus 2 (13.3%); p = 0.483 \ and restriction of elbow (12(80.0%) versus 11 (73.3%), p = 1.000]did not differ significantly between LPS and DCP group. Union of fracture site [15 (100.0%) (93.3%), p = 0.793 \ also did not differ significantly between LPS and DCP group. Functional outcome was excellent good 46.7%, fair 6.7% and poor 6.7% of patients in LPS group, whereas 33.3%, 33.3% 20.0% and 13.3% respectively in DCP group (p = 0.689) Final outcome was satisfactory in 86.7% and unsatisfactory in 13.3% patients in the LPS group66.7% and 33.3% respectively in DCP group (p = 0.39) In conclusion, Locking Plate System and Dynamic Compression Plate fixation provide equally effective method in the management of Humeral Shaft Fracture for achieving satisfactory union, functional outcome and final outcome.
